Canadian Import Permit Form (All Canadian Clients must apply for an import permit at least 5 days before collection and give us a heads up at (least 24 hours before) so we can get an appointment for the day of collection for a health certificate). We will need a copy of your approved import permit before we can proceed.
List of offices to send Permit to. Breeder located in Canada must apply for the permit. The completed application is faxed or mailed with payment ($60 canadian $) to the Import Office in the province you reside in or into which you will be importing the commodity(ies). You will be contacted by the Import Office within three (3) days of receipt of the application form with information on how to proceed.
